If your site is actively managing a COVID Outbreak, please contact your site EOC lead with any questions.
Please note:
- For any questions about the Single Site Order, please contact COVID.SingleSiteOrder@fraserhealth.ca
- Any questions related to Access, Care, and Transitions, as well as temporary absences should be referred to your ACT Coordinator. For private care homes who do not have an ACT coordinator, please direct your questions related to temporary absences or inter-facility transfers to the Access, Care, and Transitions to 604-519-8500.
- For supplies requests, Long-Term Care and Assisted Living providers are to visit the Personal Protective Equipment page and see the "How to access supplies" section.
- To report a positive COVID-19 case, complete Tools 27/28 and submit to Public Health.
