Are there eligibility restrictions?

Adults aged 19 and over with worsening of chronic health conditions, living with ongoing health issues that make living at home difficult, or being released from the hospital after surgery or sudden event such as stroke or injury. See website for additional detailed eligibility requirements.

Call for more information or to request service. Current Home Health clients should contact their local office for questions regarding their enrolled services.
Are there any fees?
Access to case managers is publicly funded and provided at no charge. Additional fees may be applied for associated supplies and services required for nursing care, depending upon client's income.
