Provides subacute care services focusing on rehabilitation and recovery to inpatients in acute care or long-term care settings who do not require specialized, intensive monitored care.

Queen’s Park Care Centre offers inpatient rehabilitation services to adults (19 and over) living in the Fraser Health region.

We have three units:

  • Stroke Rehabilitation Unit
  • Neuro Rehabilitation Unit
  • Musculoskeletal Rehabilitation Unit (MSK)

Eligibility

You must meet all of these criteria:

  • Your hospital care team has referred you.
  • You live in any community within the Fraser Health area.
  • You have had a recent medical event, for example, brain injury, stroke, fracture, usually within the last 40 days.
  • Based on your recovery so far, your hospital care team believes that more rehab can improve your chances to return to your community safely.
  • You are willing and able to take part in therapy.
  • You want help to learn how to do every day activities on your own such as dressing, bathing, eating and walking.
  • You are not yet able to live at home either with or without support in place.

Referral

Your hospital care team will refer you to our unit if you qualify for our rehab services.

Fees

Medical Services Plan (MSP) covers the cost of your stay.
