We provide highly specialized long term care and services, offering three distinct care delivery programs to specialized populations. Locally owned and operated, the Clayton Heights Care Community has many ties to the local community which provides numerous benefits for persons in care and staff.

There are 87 publicly subsidized long term care beds at Clayton Heights Care Community. All rooms are single occupancy rooms.

Room features and personalization | Our rooms include: bed, wardrobe and night table We also provide towels and linens. All rooms are single occupancy and provide a large window for natural light. Residents are encouraged to personalize their room and are welcome to bring with them: radio, TV, telephone, lamps, personal chair and pictures. |
Accessibility | Fully wheelchair accessible. We can accommodate a limited number of power mobility aids (motorized scooters/wheelchairs) |
Security | All neighbourhoods are secured with key padded entry and security cameras for safety. |
Smoking | Smoking available currently in designated area, with limitation, until such time as Fraser Health’s Smoke Free Policy implementation. |
Location information | Located within the residential community of Clayton Heights in Surrey. Local shops, amenities and public transit within walking distance. |
Additional site features | We facilitate on site access to: foot care nursing, dental care, optometry care, audiology and chaplain services. |
Parking and transportation` | Ample visitor parking and nearby public transportation. |
Programs
Social and recreation programs | Recreational Therapy Services promoting the social, physical, spiritual, cognitive and emotional wellbeing of our residents. Programing is based on residents’ abilities and interests with modifications when needed, to maximize our resident’s ability to engage. Programs are offered 7 days a week. |
Spiritual program | Weekly non-denominational services and quarterly celebration of life, facilitated by a Chaplain on site. Monthly Catholic Service with communion also provided. |
Pets | Pet visitation is welcome (with documentation) but pets living in building are not permitted. |
Medical assistance in dying | On-site assessment and on-site provision available. |

Hairdressing | Fee based beauty salon available |
Foot and nail care | Fee based, Professional foot care nursing program available |
Additional services | Occupational Therapy and Social Work services included at Clayton Heights Care Community. We also facilitate referrals for physiotherapy and companion services. Additional fee based services include: denturist and music therapy. |

- New clients can contact our Home Health Service Line at 1-855-412-2121 (open seven days a week, 8:30 a.m. - 9:00 p.m.; on statutory holidays between 8:30 a.m. - 4:30 p.m.).
- Current clients should contact your Home Health office.

Referral
Referral by case manager at a Home and Community Care office is required. To access service call the Home Health Service Line for more information; do not contact the care home directly.
Fees
There is a monthly accommodation fee; additional fees may apply for incidental services.
