Open 24 hours a day, 7 days a week.

Download our referral form for the Fraser Health Inflammatory Arthritis Clinic. Referral must be made by a rheumatologist.
The referral form must be completed in full for screening to determine if it meets our eligibility criteria. Please include all relevant medical consults and investigative reports along with the completed referral form.
Fees
No fees to attend therapy sessions.
Please note: There may be costs associated with custom made orthotics, splints, and adaptive equipment.
