At the Evergreen Hamlets at Fleetwood we specialize in care services for adults with brain injury, Huntington’s disease, multiple sclerosis and cerebral palsy.
Evergreen Hamlets focus is developing and operating communities of care focused on providing quality outcomes in a safe environment through innovation and responsiveness to the changing needs of those placed under our guardianship.
They care for the more to most difficult behaviours as a result of the disease process associated with brain injury and related diagnosis.
Evergreen Hamlets has 66 long term care beds (53 publicly subsidized). There are 50 single occupancy rooms and eight double occupancy rooms.

Room features and personalization |
Rooms include a bed, dresser, nightstand, chair, lamp and linens. Individuals can bring a computer, radio, TV, personal chair, pictures, plants and other items to make it feel like home. |
Accessibility | Wheelchair accessible and elevator-equipped, motorized wheelchairs can be accommodated. There is a lift-equipped van available for appointments and outings. |
Security | The site is equipped with key padded doors and video monitoring. Also has the ability to provide a hamlet (neighbourhood) where exiting is prevented for the safety of the residents. |
Smoking | We offer a central smoking gazebo that is situated outside the units. |
Location information | Our site it located on a bus route and with-in walking distance to a major grocery store, as well as the library, surrey recreation center and many fast food restaurants. |
Additional site features | The Evergreen Hamlets at Fleetwood have 5 hamlets (neighbourhoods) that each house 12-15 residents. |
Parking and transportation | We have limited on site visitor parking. The facility is on a busy bus route. |
Programs
Social and recreation programs | The Hamlets offers music therapy programs, therapeutic age-appropriate recreation, activity programs that may include bus trips, group outings, arts & crafts, sports & gardening. |
Spiritual program | We offer church service hosted by Fleetwood Christian Reformed Church every other Sunday. |
Pets | Personal pets are not allowed in individuals rooms, but pet therapy is offered once a month. |
Medical assistance in dying | On-site assessment is available. |
Additional services
Hairdressing | Hairdressing services are provided twice a month for a fee. |
Foot and nail care | Foot care services are provided every 6 weeks for a fee. |
Additional services | Services on site include cable vision (fee), facility telephones, physiotherapy, aqua therapy in hydrotherapy pool (fee), active rehab, cognitive neuro-behavioural intervention and in-house programs. Once a month a mobile dental clinic is offered for a fee. |

Clients must meet Home and Community Care requirements and criteria for admission. A health care needs assessment will be completed to confirm eligibility. See the eligibility requirements for home and community care section of our website.
Referral
Referral by case manager at a Home and Community Care office is required. To access service call the Home Health Service Line for more information; do not contact the care home directly.
Fees
There is a monthly accommodation fee; additional fees may apply for incidental services.
