This support service helps you stay safe, well and connected to care at home. It is part of the Ministry of Health’s Long‑Term Care at Home (LTC@Home) program. At Fraser Health, this program is called Well Connected at Home.
What is Well Connected at Home?
Well Connected at Home is a Home Health support service that helps you stay safe and independent by putting your wellness first and using easy-to-use technology to keep you connected with your care team, right at home.

- Your Home Health team are working together to support your well-being.
- Reliable and simple technology like personal alert pendants, home sensors and medication dispensers that are easy-to-use at home.
- Feeling safe, supported and connected in your own home.
How to access this service
You can access this service if you:
- are an existing Home Health client receiving services such as Home Support or the Day Program for Older Adults
- have a home that is suitable for installing the technology
- do not have a pacemaker or any other implanted device
- have someone involved in your care whom we can contact when you need help
