Fraser Health’s communities are diverse, growing and unique, and health care services need to reflect the people and communities they serve. By strengthening connections between communities and health care services, we are better able to support the health and well-being of people across the region.
This community-focused approach helps ensure services are planned and delivered with a deeper understanding of local needs, priorities and population growth. Stronger connections between hospitals, primary care, community services, mental health and substance use supports, home health, long-term care and public health programs help people experience more coordinated care closer to home.
