Home health care rehabilitation services are arranged through the Burnaby Home Health office located on the fourth floor; located in the same building as the Burnaby Public Health Unit.

Eligibility

Your Home Health care provider may recommend programs to you or you may be able to apply directly for access. Learn more about accessing Home Health services.

You must meet eligibility requirements for Home Health services prior to accessing services. Learn more about who is eligible.

Referral

For more information or to request service, call the Home Health Service Line. Ask for a care needs assessment by a case manager. Referrals are accepted from the client, family members, hospital, a family physician, friend or neighbour, community groups or government agencies.

Fees

Some services are provided free of charge while others include a cost based on your after tax income. Learn more about costs associated with our Home Health services.

Language (s) Offered

Primarily English. Translation services can be made available if sufficient notice is given. It is suggested the client arrange to have an interpreter present if understanding English is difficult.
