This clinic is operated by nurse practitioners and physicians providing a wide range of health care services for the post-discharged care of patients/clients without a family physician in the community and government assisted refugees.
Hours of operation
Tuesday: 8:30 a.m. - 4:30 p.m.
Wednesday: 10:00 a.m. – 6:00 p.m.
Thursday: 8:30 a.m. – 4:30 p.m.
Saturday: 9:00 a.m. – 5:00 p.m.

Referral
- Patients/clients recently discharged from the Abbotsford Regional Hospital who do not have a family physician in the community can be referred.
- A referral can come from a hospitalist, community agency, or other health care providers.
- Our nurse practitioners and physicians provide temporary post-discharge follow-up care through physician referral.
- Government assisted refugees can be referred through community agencies.
