Day Program for Older Adults provides social connection, fun activities, exercise, and health checks for seniors who have health challenges and want to keep living as independently as possible.

Also known as the Creekside Day Program and offers the therapeutic bathing program. Participant eligibility to the program is determined through the local home health office.

Monday - Friday, 7:30 a.m. - 3:30 p.m.
Closed statutory holidays. Hours may vary, please call ahead to confirm.
