Referral- based clinic, using a multi-disciplinary approach to manage various types of chronic pain. This clinic aims to use a holistic approach that addresses the patient's physical, emotional, mental and social factors that contribute to their pain.
The team works in collaboration with the patient and the referral sources to help improve the lives of people living with chronic pain. This clinic provides access to services from pain specialists, nurses, physiotherapists, occupational therapists, social workers, pharmacists and psychologists as needed. Services may include a combination of pain science education, physical therapy, cognitive-behavioural therapy and lifestyle counselling, medication management and interventional procedures.
Each patient is given an individualized approach to their goals and needs, with a focus on goal attainment, self-management and functional improvement.
