The Anticoagulation Management Clinic is run by clinically-trained pharmacists who deliver comprehensive care to patients who are newly initiated on anticoagulation treatment.

The clinic offers short-term, pharmacist-led anticoagulation support for patients who have already been initiated on therapy by their referring provider. Services include:

  • Initiating an anticoagulation plan as directed by the referring provider (Note: The decision to start anticoagulation is made by the referring physician, not the ACM pharmacist).
  • Pharmacist-led management and education on warfarin and/or low molecular weight heparin.
  • Monitoring patients initiated on warfarin who require close follow-up due to:
    • Significant drug interactions.
    • Temporary unavailability of their primary care provider.

Duration of care: The clinic provides support for up to four weeks, or until the patient achieves a therapeutic international normalized ratio (INR), whichever occurs first.

For patients requiring bridging orders, patients will need to be referred to Internal Medicine Services

Eligibility

Although most patients referred to the clinic are discharged from Surrey Memorial Hospital and Fraser Valley Cancer Clinic, any patient can be referred to this service if they meet the following criteria:

  • Lives south of the Fraser River.
  • Has newly diagnosed thrombosis.
  • Patients with left ventricle (LV) or apical thrombus must be referred by a specialist, have follow-up appointment arranged or will not be accepted.
  • Is initiated on warfarin or other oral or parenteral anticoagulant.
  • Is able to attend multiple clinic appointments at Jim Pattison Outpatient Care and Surgery Centre.
  • Is willing to be an active participant in their care.
  • Can be contacted by phone to facilitate follow up.

Patient must have primary care provider or be registered with Health Connect Registry for ongoing anticoagulation care.
