Provides short-term primary care services for adults connected to Fraser Health Mental Health and Substance Use services who do not have a primary care provider and have complex medical needs.
The Primary Care Clinic is designed to support adult patients (ages 19+) who are connected to MHSU services and do not have an existing primary care provider. It primarily serves individuals with complex medical needs that require primary care support beyond the services provided by psychiatrists and the clinical MSHU team.
The clinic delivers short-term, comprehensive primary care services alongside health promotion and education.
Patients receive services through the clinic located within their community. This means that individuals are supported by the Primary Care Clinic in the same geographic area as their MHSU team (i.e. Surrey clinics serve Surrey patients, and Burnaby clinics serve Burnaby patients).
Clients can expect:
- Assessment, treatment and follow-up
- Health promotion information
- Referrals to other services
- Consultation to other GPs in the community
- An integrated approach to services delivery through coordination of care with mental health services, hospital and other community agents
Services from the Primary Care Clinic are offered in partnership with mental health and substance use addictions services.
Access
Eligibility
To be eligible for services at the Primary Care Clinic, patients must:
- Be connected to a Fraser Health MHSU team within the clinic’s designated community
- Not have an existing primary care provider (physician or nurse practitioner)
- Present with complex medical needs requiring primary care support
Referral
Referrals to the Primary Care Clinic may be submitted by the patient’s treating psychiatrist or Most Responsible Clinician. The Primary Care team will review each referral and follow up with the referring provider to confirm whether the referral has been accepted.
