Primary Care Network (PCN) clinical pharmacists ensure medications are appropriate and effective for your medical condition.
They will:
- Spend dedicated time with patients to learn about and address their health needs and goals
- Help patients understand their medications so they can take an active role in their care plan
- Address barriers to medication use
- Manage side effects, polypharmacy, and adherence concerns
- Support medication changes, including deprescribing as appropriate
- Help manage chronic conditions
