Freedom of Information (FOI) requests

BC's Freedom of Information and Protection of Privacy Act (FOIPPA) allows the public the right to request a copy of any record within the custody or control of Fraser Health. The spirit of FOIPPA is to promote transparency and trust of Fraser Health by the public. A record is defined as any form of recorded information, whether physical, digital, written, or video. Fraser Health has a legal duty under FOIPPA to assist all requestors and respond openly and in a timely manner.

Make a Freedom of Information request

Please note, there is a $10 fee for making a Freedom of Information request for records that are general in nature. There is no charge for making a request for records that are personal e.g. employee records. This fee is being charged in accordance with s.75(1)(a) of the Freedom of Information and Protection of Privacy Act (FIPPA). If you have any questions about the fee implementation, the FOI process, or need assistance making your request, you can send an email to FOI@fraserhealth.ca, or leave a voicemail by calling 236-484-1479.

A Freedom of Information request does not replace any existing method of requesting records. If a record can be obtained via another method, your request will be re-directed to that service. Information on other types of record requests can be found below. Health records cannot be requested under a Freedom of Information request, see below for details on how to request a copy of your health records.

Health records request

A health record can include clinical and medical records, lab results, CT scans and more. Visit the request a health record page to learn more information on how to request your records.

Employee records request

Current employees can request a Verification of Employment letter, or Record of Employment via the MyFH portal under the Request my Information section. Current employees can contact People Information and Benefits to request a copy of their personnel file.

Previous employees can seek a copy of their personnel, payroll, disability management, performance management, and/or human resources related records through the FOI office: FOI@fraserhealth.ca.

The FOI Team cannot assist you with the following request types: Generating a record of employment (ROE) or verification of employment letter, completing a Form 37, or completing/creating other records to support applications, such as, mortgages or job applications.

Proactive disclosures

We publish a variety of information throughout this site including financial and performance information, inspection reports, outbreak information, and select policies and procedures.

For example, this Accountability section includes information about our finances and the work we do to monitor our performance to ensure our services meet your needs.

The information on this site is proactively disclosed in accordance with our transparency and access to Fraser Health records.
