How to access our services
New clients
Contact our Fraser Health Access Line: Home and Community Care (previously called the Home Health Service Line). There are 2 ways to do this:

- Book a telephone appointment by filling out our online form.
We are open 7 days a week from 7:00 a.m. to 9:00 p.m., 365 days a year.

Our caregiver support clinicians are social workers who provide a wide range of caregiver support and services to current home health clients.
This includes:
- Support for caregivers experiencing stress, burn-out, grief and/or loss.
- Resources to prepare for advance care planning.
- Support for people who are new to the caregiver role.
- Caregiver support groups.
- Advocacy for caregiver/client rights.
