Eligibility
- We provide sleep services for patients aged 17 and over.
- We accept patients from the Fraser Health Region and across British Columbia.
Referral
A referral by family physician or specialist is required for this service.
General practitioner (GP), nurse practitioner (NP) and other specialists:
- Patients will receive consultation to Sleep Specialist.
- Please complete the Abbotsford Regional Hospital and Cancer Centre Sleep Clinic Referral Form provided below. Select one sleep specialist of your choice and fax completed form directly to their office.
