The Thrombosis Clinic provides specialist physician assessment for patients with confirmed venous thromboembolism (VTE).

This clinic is intended for medical evaluation and follow-up of venous thrombosis, not for routine anticoagulation management of arterial cardiovascular disease. If patient requires short-term pharmacist-led anticoagulation management while on warfarin and/or Low Molecular Weight Heparin (LMWH), please refer to Anticoagulation Management.

Access

Eligibility

Any patient can be referred to this service if he/she meets the following criteria:

  • Lives south of the Fraser River.
  • Has confirmed VTE.
  • Is willing to be an active participant in their care.
  • Can be contacted by phone to facilitate follow up.

Note: Initiation of anticoagulation therapy, if indicated, should be done prior to referral.
