Kidney Care Centres promote early identification of renal disease and provides preventative care and education to delay or avoid a future need for dialysis.
The Kidney Care Centre also provides support to patients during transition into hemodialysis, peritoneal dialysis or transplant.
Patients are offered education and monitoring of diet and kidney function, with an emphasis on patient self management.
Upon receiving a nephrologist referral, the centre mails a registration package to the client to complete and bring to the first appointment, along with a valid CareCard and current medications.

- Must have a GFR (glomerular filtration rate) of 45 or less
- Must be 19 and over

Referral from a nephrologist is required.
