Our Home Health clinics provide nursing care and other professional services for clients who can access services outside of their home.
This support service helps you stay safe, well and connected to care at home. It is part of the Ministry of Health’s Long‑Term Care at Home (LTC@Home) program. At Fraser Health, this program is called Well Connected at Home.
