How to access our services
New clients
Contact our Fraser Health Access Line: Home and Community Care (previously called the Home Health Service Line). There are 2 ways to do this:

- Book a telephone appointment by filling out our online form.
We are open 7 days a week from 7:00 a.m. to 9:00 p.m., 365 days a year.

What can you expect?
Palliative services are provided in the home, hospital (i.e. palliative complex care unit), in a long-term care facility and/or at hospice.
Our services include:
- Grief and bereavement counselling.
- Prescribe and support the use of equipment.
- Access to pain management services.
- Support for loved ones and/or family members.
