Public Health regularly reviews immunization records for students to help make sure they are up to date on vaccines.
Public Health offers all routine vaccines needed for free to students in Grades 6 to 12. The vaccines are provided at school.
How do I know if my child is up to date
Parents can review their child’s immunization records online by logging into immunizationconsent.gov.bc.ca and looking at “Immunization history” to see a report of their vaccine history and then going to “Immunization forecast” to see what vaccines are needed.
What if my child had vaccines, but they are not on their immunization record?
If the child’s record is missing vaccines that they have already had, please do the following:
- Find the paper record at home, if given to you at the time of vaccination.
- If you do not have records at home, contact the health care provider who gave the vaccines and ask for a copy of the immunization record.
- Report the immunizations either of these ways:
- Include it with the signed consent form and return it to the school sealed in the envelope provided.
- Submit it online by going to FraserHealth.ca/immunizationform.
Providing consent for immunizations
Complete either the online consent form or the paper consent. You do not need to fill out both.
Before providing consent, please complete the following steps:
- Review the immunization history that is provided for the child online or in paper format.
- My child or the child in my care missed the school immunization clinic. How do I get them immunized?
If Public Health is coming back to the school, a notice will be sent out with the date they are coming back.
Online consent for students in Grades 6-12
Parents, students (in grades 9 to 12) or caregivers have the option of providing consent for vaccines online. You can fill out consent any time before the school clinic.
To give online consent for immunizations, go to immunizationconsent.gov.bc.ca.
Parents, students or caregivers who complete the online consent will receive a letter closer to the time of school immunizations so they are aware of the date.
If consent is not completed online, students will receive a letter and paper consent form from the school close to the time of immunization. The letter will say when the consent needs to be returned by.
Paper consent for students in Grades 6-12
- If you are not able to do consent online or choose not to, you can sign the paper consent form that will be sent to you.
- Return the signed consent form to the school, sealed in the envelope provided, by the date stated in the letter.

I did not get a consent form for my child or the child in my care. What do I do?
You can fill out the consent form online for your child at immunizationconsent.gov.bc.ca or call your local Public Health unit to ask for a paper copy to complete.
