Service is for Chilliwack, Agassiz, Harrison & Hope community residents living in their home, who have medically complex issues.
For patients 65 and over. Patients under 65 years of age are considered on a case-by-case basis.

This service requires a referral from a physician, nurse practitioner or health professional.
