Checkbox Survey Tool is being used by Fraser Health to collect data from users wishing to request transportation to a Fraser Health vaccine site. Information collected will allow Fraser Health to provide transportation service to Fraser Health clients.
What personal information is being collected?
Your personal information collected by this Checkbox form is under legal control by Fraser Health and is subject to protections under the B.C. Freedom of Information and Protection of Privacy Act (FIPPA). To participate, you are being asked to enter the following information for use by Fraser Health:
- First and last name
- Phone number
- Community of residence
- Vaccine confirmation number
- Date of birth
- Comments (optional)
Who will have access to the personal information?
The information collected will be used by Fraser Health to inform the Transportation team of which clients are requesting assistance. At no point in time will your personal information be disclosed for purposes other than providing transportation services.
Where will the personal information be stored?
Your information will be stored on Checkbox’s server, located in Montreal, Canada, and will be protected by Checkbox Survey Tool in compliance with their data protection policy.
How long will the personal information be stored?
Your information will be retained by Checkbox Survey Tool until Fraser Health staff removes the data, once your appointment date passes.
