Provides specialized services to older adults experiencing multiple medical issues and who may be at risk of losing their ability to live independently.
The Specialized Seniors Clinic does not replace your family doctor or nurse practitioner and usually does not manage long-term follow-up.
Specialized Seniors Clinics (SSCs) are secondary outpatient health services. Services are provided by an interprofessional team with expertise in geriatrics who consult on frail or at risk for frailty older adults with complex health needs where primary care providers have exceeded or exhausted their existing expertise in geriatric care.
The goals of the SSC services are to support older adults’ quality of life, improve ability to manage chronic conditions , maintain independence and minimize risk for dependency.
