The Community Chronic Pain Program is a comprehensive eight-week self-management program. Now accepting referrals for all patients living with persistent pain.

What kind of services are available? 

Please note: Due to COVID-19 the eight week program is now available virtually and can be completed from home.

Eight week self-management program provides:

  • Group sessions on pain.
  • Learn and build self-management skills and tools to decrease pain and improve quality of life.
  • Group exercise activities and individual physiotherapy and occupational therapy care.
  • An extended pain team review, including a pain specialist and pharmacist for those who need it.
  • Assisted referral to more pain, mental health and substance use services if needed.
  • Awareness and assistance connecting to community resources to support ongoing self-care.
  • An individualized ongoing management plan created with the client. This will be shared with the primary care provider for ongoing support and care. 

The commitment to complete this program is 2.5 hours per week for eight weeks.

Eligibility

Adults 18 years and older with persistent pain and interested in self-management skills and able to participate in group learning. 

Referral

Adults living in the Fraser Health region can be referred to the program by their primary care provider or the Fraser Health Mental Health and Substance Use Services.
