Outpatient Rehabilitation Services enable clients to comfortably self-manage their rehab goals and support them with a smooth transition back to the community by providing early intervention post-discharge from the hospital to maximize function and quality of life in the community. You are welcome to bring someone with you to your rehab sessions.

Rehabilitation (rehab) is a set of therapies that help you improve your physical, cognitive (thinking) and emotional function after an injury. Rehab units provide physical, communication and cognitive (thinking) rehab to advance your recovery.

Your rehab care team is dedicated to providing person-centred rehab. We are committed to creating a positive environment that includes respect, teamwork and clear communication. We are united in our passion to help people and families achieve their goals.

Note: People often refer to rehabilitation as ‘rehab’.

The referral form must be completed in full for screening to determine if it meets our eligibility criteria. Please include all relevant medical consults and investigative reports along with the completed referral form.

Fees

No fees to attend therapy sessions.

Please note: There may be costs associated with custom made orthotics, splints, and adaptive equipment.
