Cari Hoffman

Cari Borenko Hoffmann has been the regional coordinator for advance care planning in Fraser Health since 2006.

Cari has supported, and facilitated, the development of disease and program specific advance care planning education with Fraser Health’s Renal, Residential Care and Assisted Living, Older Adult, Mental Health and Substance Use and Cardiac programs. She is the chair of the National Community of Practice, Educators Canada and a member of the National Task Group. Cari has provided support to clinicians and organizations around the world. Professionals from Canada to New Zealand, to Singapore and Japan have benefited from Cari’s expertise; and adapted their advance care planning programs and materials as a result.

She has been featured on the radio show CBC Almanac, as well as in newspapers including the Vancouver Sun, National Post, Burnaby Newsleader and Chilliwack Progress. Articles have also been published in the Sounding Board and Hospital News. Cari is the provincial clinical lead for advance care planning with the B.C. Centre for Palliative Care and is a clinical instructor at UBC Department of Medicine.

In 2014, Cari authored the chapter Passion, Persistence and Pennies in the book Advance Care Planning: Communicating About Matters of Life and Death. She presented at the International Conference on Advance Care Planning in London, England and has provided information to health care professionals at a variety of conferences including Canadian Hospice Palliative Care, BC Association of Social Workers, BC Renal Days and BC and Ontario Hospice Palliative Care. Cari has also presented to health care professional at the BC Cancer Agency, ALS Society, Canadian Network for Asthma Care, as well as community groups such as rotary clubs, probus, breast cancer support group, stroke club and a variety of church affiliated programs.
