This means you will be given the information you need to make the best choices and decisions for you. You and your health care providers will manage your care together.

Being person-centred means we will do the following:

  • Treat you with respect and dignity
  • Share the information you need to know
  • Encourage you to be active in your care
  • Ask for your thoughts and ideas for improving the health care system

Person-centred care leads to:

  • Better health for everyone
  • A positive health care experience for you
  • A positive health care experience for staff and medical staff

“What matters to you?” is a simple question that can have a big effect.

What is the most important thing you want us to know about you?

We want you to share what matters to you with your care provider.

By sharing what matters, you and your health care provider can make sure your care meets your needs.

Being asked “What matters to you?”  and sharing what matters to you are examples of person-centred care.

Our Shared Commitments are the promises we make to you. They are the promises we ask you to make to us.

Together, patient partners and health care professionals wrote Our Shared Commitments.

These commitments reflect our promise to deliver health care that meets your needs. Our Share Commitments are person-centred.

This guide was written by Health Quality BC. It has tools to help you talk to your health care provider about what matters to you. You will find these tools on page 16. This guide also has tools for health care providers.
