The health of our communities is influenced by the social, environmental and economic conditions in which we live. These are often called the social determinants of health and include factors such as: education, income, supports during early child development and access to safe and affordable housing.

Population health is an approach to health that aims to improve the health of our communities through action on the social determinants of health.

Partnerships to Health: Fraser Health Medical Health Officers Annual Population Health Report

The purpose of this report is to help us understand why some groups of people are not as healthy as others and how we can all work together to improve health. Improving the health of a population requires sustained collaboration and partnership between Fraser Health and the individuals and communities it serves, other organizations and all levels of government.

The Health Profile (formerly known as the Population Health Supplementary Report) provides an overview of the health of our population, focusing on non-medical determinants of health and health status indicators.
