Age: 19 and over
Details: For home care nursing or physical therapy, clients must meet standard health care eligibiity; require care following discharge from a hospital, care at home rather than hospitalization, or care because of a terminal illness; and have a local doctor. For subsidized home support or day programs, or to receive the services of a case manager, clients must: be 19 years of age or older; meet standard health care eligibilliity; be unable to function independently because of chronic health-related problems or have been diagnosed with an end-stage illness.
