Home Health offers a variety of at-home and community services to people with acute, chronic, palliative or rehabilitative health care needs

When would I call the Home Health Service Line?

Call if you have concerns such as these:

  • I think my dad needs help and I don’t know whether he can stay at home or should move to a residential care facility
  • Who will help my Dad with his personal care when I'm at work?
  • My husband has been in hospital and is ready to come home. Who can help me plan for that?
  • I don't think I can live alone anymore
  • I think I need some help but I don’t know what help I can get
  • I can't care for my mom on my own anymore. I need help
  • My mom is starting to have trouble bathing herself
  • I’m worried that my mother is not taking her medications properly and is getting forgetful

What services does the Home Health program provide?

We offer both short-and long-term services. Short-term support helps you recover from a hospital procedure or condition. Long-term programs provide ongoing care that will give you the independence you need to remain at home comfortable.

The types of services we offer include:

  • Home support services for personal care needs (e.g. bathing, toileting, feeding and/or grooming)
  • Clinics for wound care in all communities for clients who are able to get about in their community
  • Case Managers and nurses who work closely with community doctors to support seniors and individuals with complex functional and care needs
  • In-home nursing or therapy care for adults who are unable to leave their home
  • Day programs for seniors to provide therapeutic opportunities for socialization, mental stimulation and increased strength and mobility as well as to provide respite for caregivers
  • Respite beds to provide caregivers with some time away from the intensity of caregiving

What can I expect when I call?

  • Home Health Service Line staff do their best to answer every call as it occurs. Occasionally the volume of calls during peak hours may mean you receive a voicemail message asking for a callback number, which will be returned by 4:30pm
  • We address each call individually as each situation is different. Our customer service staff will answer your questions to determine which of our services best addresses your needs
  • If you require service from a nurse or other health professional, they will contact you as soon as possible to arrange a clinic visit, or, if you are unable to travel to a clinic, a home visit
  • If you require home support this would be discussed upon your first visit with a Home Health professional

Am I eligible for Home Health services?

You must meet eligibility requirements for Home Health services.

How soon will I receive service?

  • Our Home Health staff will work with you to determine your best care options. Our approach to meeting your health care needs is guided by the following principles:
    • Everyone should have the information they need to make decisions about lifestyle and care
    • Services will complement and supplement, but not replace, your efforts to care for yourself
    • You have the right to make your own care decisions, including the right to live with risk
    • Services will promote your well-being, dignity and independence
  • We do our best to meet the demand for our services in a timely way and often need to set priorities due to high demand
  • Just as in an Emergency Department in a hospital, we prioritize the delivery of our services based on a professional assessment of the needs of each client
  • The Service Line clinician on the phone will ask several questions to help them best establish the priority for each client
  • Sometimes, if we are unable to provide immediate service, the clinician will suggest interim options for care while you are waiting for Home Health

What is the cost of services?

  • There is no charge for professional services such as nursing or occupational therapy
  • If you need home support services, we will conduct a financial needs assessment, at no cost to you, and, depending on your income, there may be a charge for home support services.

  • We anticipate adding email to our communications in the future but at this time we do not have the ability to manage public emails.

Can my doctor arrange these services for me?

  • Yes, your physician, Nurse Practitioner or GP office would call the HH Service Line and follow the telephone prompts for physicians and other health care professionals.
