Working to improve the health of the population and the quality of life of the people we serve.
There are times when you find you cannot manage an illness or condition alone. Knowing what options are available will help you, your family and friends maintain your care more independently.
Short and long term health care
In this section you will find information about how to access services provided in the home, in clinics and in community settings. Home health services complement the care people can provide for themselves. The type and amount of services depend on each client’s care needs and eligibility.
Alternate living arrangements
You can also find out about options for alternate living arrangements, such as convalescent care, assisted living and residential care, when you are no longer able to safely manage living at home.
You can find out about support for caregivers, hospice palliative care, planning for future health care and accessing medical equipment and supplies.
See the menu items under Your Care on the left to learn more about the services available.
