• Many of our Home Health services complement the care that people can provide for themselves, and the help they may get from family, friends and community.
• Our Home Health services are designed to support your independence whether the services are provided in the home, in clinics, or in community settings.
• Home Health services may be available on a short-term or long-term basis. The type and amount of service depends on each client’s care needs. There are minimum eligibility requirements for all Home Health Services.
