The first step to entering a publicly subsidized residential care facility is to have a health care needs assessment completed.

We make it a priority to provide assessments to people with the greatest need.

How do I access residential care if I am living at home?

You can access this service through a Home Health case manager who will determine your needs and eligibility. Priority is always given to those with the highest need and at the greatest risk.

How long does it take to access a facility?

The search for a residential care bed that best meets your needs starts as soon as the case manager completes your assessment. We aim to locate the first available bed in your home or neighbouring community.

It is important to let the case manager know if you experience changes in your health while waiting for a bed.

How should I prepare to move into a facility?

The time between being notified of a vacancy and moving into a residence is often very short. Therefore, it is necessary to organize for the move in advance. Read the Preparing to Move page for more information.

What if I want to change facilities?

Ifthefirst available bed vacancy is not in the location you want, you can make arequest to move once you have settled in. When a newbed becomes available in your preferred location, you will have the choice of accepting or declining it.

How do I access residential care after a hospital stay?

If you are assessed as needing residential care following a stay in the hospital, depending on your care needs you can:

  • Return home with appropriate care and services in place while waiting for a suitable publicly subsidized residential care be
  • Move into a private pay residential setting while waiting for a suitable publicly subsidized residential care bed
  • Move to a Transitional Care Unit while waiting for a residential care bed if it is not possible to go home, even for a short period. In this case, the daily residential care rate will be charged as if you are already living in residential care
