How to access our services
New clients
Contact our Fraser Health Access Line: Home and Community Care (previously called the Home Health Service Line). There are 2 ways to do this:

- Book a telephone appointment by filling out our online form.
We are open 7 days a week from 7:00 a.m. to 9:00 p.m., 365 days a year.

A community health professional completes an assessment for the following services:
- Assisted living
- Advance care planning
- Caregiver support clinicians
- Community health nursing
- Day program for older adults
- Home support
- Long-term care
- Nutrition and dietitian
- Long Term Care and Assisted Living Overnight Respite Care
- Palliative Care
- Rehabilitation therapy
- Seniors' community connector
- Social work services

Referral
Current clients should reach out to their local Home Health location listed below. Your health care provider may also recommend our services. For information or referrals, contact 1-855-412-2121 (toll-free, open seven days a week, 7:00 a.m. - 9:00 p.m., 365 days a year).
Fees
Some services will have no cost to you, while others will require you to provide your tax information to access services.
